What the inspector wrote

On 08/05/2025, Licensing Program Analyst (LPA) Randy Miranda conducted a case management visit for an incident that occurred on 06/09/2025. Present during the inspection were two (2) directors, (8) staff members, and fifty-one (51) children in care. On June 9, 2025, the facility self-reported an incident involving a child (C1) who was grouped with other children and a teacher to use the restroom. As the group proceeded toward the restroom area, C1 ran ahead, tripped on uneven pavement, lost balance, and struck a wall. The teacher who witnessed the incident reported that C1 was unable to brace himself with his hands before impact. C1 was promptly treated for a scraped face and a nosebleed. Staff were immediately notified and administered care for the facial injuries. The child\342\200\231s parents were contacted, and the father arrived to take C1 home. He later stated that C1 was taken to Kaiser Permanente for precautionary evaluation. Although no treatment was necessary, C1 was advised to rest at home. The wall C1 collided with contained embedded stones. The father expressed concern about its surface, prompting Adventure Time to escalate the matter to the school principal and district. Staff have since been positioned in the area to remind children not to run near the wall or over uneven pavement. No deficiencies were issued during today's inspection. A notice of site visit was given and must remain posted for 30 days. Exit interview was conducted with director Tammy Brewer.
