What the inspector wrote

On February 26, 2026, at 11:00am, Licensing Program Analysts (LPA) Randy Miranda arrived unannounced for an annual inspection for compliance to health and safety for children in care and met with Director Ralph Thomas. Present for the inspection was the site director, three staff members, and 18 school-aged children in care. The center currently operates Monday through Friday from 7:00am to 5:30pm; however, due to zero enrollment for the morning session, the facility begins the day at 11:00am. Ages served range from kindergarten to 14 years old. The total capacity is 60. The school operates in a portable classroom on the Chabot Elementary School Campus. The facility is clean, well-organized, and equipped with ample age-appropriate furnishings and equipment. All surfaces, including floors and countertops, are clean and free of toxins. Hazardous items and toxins are securely stored in a locked cabinet. There are no bodies of water accessible to children in care. All documents required to be posted were appropriately posted on the walls: License; Emergency Disaster Plan; Earthquake Preparedness checklist; Notification of Parents Rights; Personal rights; Child Seatbelt laws; menus; daily activity schedules. The facility has one fully charged 3A40BC fire extinguisher in the portable classroom, located at the main entrance door. The last fire system inspection was conducted on 07/17/2025. The combination smoke/carbon monoxide detector was not tested because it is wired to the local fire department dispatch. Disaster drills are conducted at least once every six months; the last drill was conducted on 12/11/2025. Heating and ventilation in the classroom are acceptable. Outdoor play activities take place on the Chabot Elementary School playground. The outdoor play structure has cushioned material underneath to help reduce injuries from falls. The structure is sturdy and safe for children to use. Shade is available throughout the school\342\200\231s building eaves and outdoor hallways. Fresh drinking water is readily available, and teachers provide continuous supervision. The center provides snacks, and children bring their own lunches from home. Dry goods and canned foods were checked, and all items were within their expiration dates. Children and staff use the school restrooms, with separate restrooms designated for staff. Children\342\200\231s records were reviewed and found to be in good order. Staff files were also examined, and three Mandated Reporter Training certificates were identified as needing renewal. An IMS plan, along with the original prescription, was attached to an epinephrine auto-injector. Site Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). Children bring their own water bottles, and the school also provides bottled water to children. Lead testing is complete and documented. This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of \342\200\234medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS information see PIN 22-02-CCP.
