What the inspector wrote
On October 6, 2025, at approximately 8:00 AM, Licensing Program Analyst (LPA) Elimika Woods conducted an unannounced Annual/Random Inspection and met with Director Yolanda Garnett. LPA disclosed the purpose of the inspection and toured the facility inside and outside to conduct a Health and Safety inspection. During the inspection, there were twenty-six (26) preschool-age children present, along with seven (7) staff members, twelve (12) Chabot College Early Childhood Education (ECE) lab students, and four (4) Chabot College Student Assistants. The facility operates in Buildings 3500 and 3700 on the Chabot College campus and is a Title V program. The hours of operation are Monday through Friday, from 7:30 AM to 5:00 PM. Two (2) classrooms in Building 3700 (PreK4 and PreK5) are not in use Monday through Friday but are utilized for a separate Saturday program.
All individuals subject to criminal record review have obtained clearance or an exemption and are associated with the facility. LPA conducted a census of the children, which matched the twenty-six (26) electronic signatures recorded in the electronic sign-in system and on the sign-in sheet. At approximately 9:00 AM, LPA reviewed five (5) children\342\200\231s files. Each file contained the required documents, including Personal Rights, Parent\342\200\231s Rights, Medical Consent, and Identification and Emergency Information forms. At approximately 9:30 AM, LPA reviewed four (4) staff files. All staff files contained the required Health Screening and Employee Rights forms. At least one opening and closing staff member has current CPR and First Aid certification.
LPA tested the carbon monoxide detector and found it to be functioning properly. The center has wired smoke detectors connected to a system that notifies the fire department when activated. Per the director, the smoke detectors are fully functional and serviced regularly. The center also has a working telephone, fully stocked first aid kits, emergency backpacks, and multiple fully charged fire extinguishers (types 2A10BC and 3A40BC) mounted on the walls in each classroom, meeting the standards established by the State Fire Marshal.

At approximately 8:15 AM, LPA inspected the three (3) classrooms and observed teacher-to-child ratios in compliance. Classrooms are arranged to allow children to explore various learning areas. There are adequate play and educational materials available. Floors were clean, furniture was age-appropriate, and equipment was observed to be in good condition. Individual cubbies are available for children\342\200\231s personal belongings. The designated isolation area for ill children is located in the front office, separate from other children in care. All required postings were observed in a publicly accessible area.
This facility provides breakfast, lunch, and snacks for children in care and there are weekly menus posted at the facility. Low fat milk and non-fat milk is being served to children in care. LPA observed an area where they have a mini refrigerators those areas are clean and free of evidence of rodents. LPA reminded the director that all persons engaged in food preparation and service shall observe personal hygiene and food service sanitation practices to protect food from contamination. The kitchen area is off limits to children in care and its locked and inaccessible
At approximately 10:15 AM, LPA inspected the staff bathroom and was informed by the facility representative that it is separate from the children\342\200\231s bathrooms. The children\342\200\231s bathrooms are located inside the classrooms. LPA checked and flushed all toilets in each bathroom and confirmed that they were in proper working condition. LPA also tested the faucets to ensure the water temperature was safe and appropriate for children\342\200\231s use. The water maintained a comfortable temperature, and all plumbing fixtures were observed to be functioning as designed during today\342\200\231s inspection.
This facility provides Incidental Medical Services \342\200\223 IMS. . LPA reminded the director that a refrigerator shall be used to store any medications that requires refrigeration.
LPA Observations During Inspection-
At approximately 8:20 AM, LPA observed children in each of the three classrooms eating breakfast at the tables. Staff were seated at the tables engaging in conversation, while students from the Chabot College Childcare Lab Program were present, observing the children and taking notes. Most lab students were seated at nearby tables positioned to allow clear observation of the children. Lead teachers were strategically positioned to supervise both children and staff, providing guidance and support as needed. The children were well-behaved and engaged during the observation. A few children asked LPA about the visit, and LPA kindly explained that the purpose of the visit was to make sure they and the center were safe.

The outdoor playground area was observed to be fully fenced and free of defects or hazardous conditions. The playground contains multiple trees that provide ample shade for children during outdoor play. All play equipment was age-appropriate, securely anchored, and in good repair. The area includes tables, chairs, building blocks, planter boxes, drawing easels, and climbing structures. Two storage sheds were observed on the playground; both were locked and inaccessible to children in care.
California Law requires Child Care Centers licensees to
report unusual incidents
or injuries to children in care to child's parents and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624). Incidents must be reported within 24 hours by phone, fax, or electronic mail. The director was also reminded that Mandated Reporter Training ("General" and "Child Care Providers") is required for all staff and is to be renewed every 2 years by visiting

Lead Testing \342\200\223 Child Care Centers (CCC) CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES: Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

The director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
The director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
There are no deficiencies cited today. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the director
