What Happened

The analyst conducted an unannounced case management visit after an unusual incident in which a child was left alone in the classroom during transition time. Staff reviewed the transition process and completed biannual training focused on name-to-face checks and counting during transitions, allowing the deficiency to be cleared the same day.
