What the inspector wrote

On 02/07/2025 at approximately 9:30am, Licensing Program Analyst (LPA) Lorraine Dacanay Breaux conducted an unannounced case management inspection for the purpose of investigating a reported incident. LPA met with Director Lindsay Anderson. Also present during the inspection were four (4) fingerprint cleared staff and 31 children in care for this component. (Upon arrival LPA had to wait outside for several minutes, LPA was allowed entry by the cook, per Lindsay was providing a breaks to staff making and/or ensure the classrooms were within ratio, LPA waited 10-15 minutes). The facility has an infant component #013412607. The reported incident occurred on 01/07/02025 and was reported to CCLD by the director on 01/08/2025. Per director, around 4:30 PM (end of the day) during transition from the play ground to Preschool A classroom, two (2) of the preschool children went back outdoors without the teacher noticing. The teacher closed the door, while still in classroom C the teacher notice 2 children were not present. The teacher from Preschool C asked where a specific child was and then she opened the classroom door and noticed two (2) children on the play yard. Per director the children were outside less then 1 minute when transitioning indoors from the outdoor play area. During today’s inspection, LPA reviewed the above information with Director and conducted an interview to obtain more information. Both teachers were unavailable during today's inspection. The facility was toured to conduct a Health and Safety Inspection of the classrooms where the child was reportedly unsupervised. During the incident, the child was transitioning from the outdoor play area to the classroom. Present was (1) teacher and twelve (12) children for preschool A. This classroom entered into Preschool C classroom which was one (1) teacher and twelve (12) children. After this incident on 01/7/2025, the director spoke with both parents of the children. On 1/15/2025 at the staff meeting the staff underwent retraining, the agenda included the following topics: Staff Child Supervision Record, name to face checks enforced, transitions, safety and supervision training. To ensure accountability, director will conduct random transition observations. This incident will also be reviewed during the monthly staff meeting. Based on the information obtained and LPA’s observation, LPA determined the children were left unsupervised, which posed an immediate threat to the health and safety of the child in care. Absence of supervision is a Zero Tolerance violation. As a result, a Type A deficiency is issued during today’s inspection. (see 9099-D) As a result of the Type A deficiency cited, a copy of this report and LIC 9224 Acknowledgment of Receipt of Licensing Reports must be provided to parents/guardians of current enrolled children in care and all children newly enrolled following a 12 month period of this report. The LIC 9224 must be signed within the next business day the children are in care and is to be kept in the children’s files. A Notice of Site Visit was given and must remain posted for 30 days.
