What Happened

Unannounced incident investigation revealed child left alone outside on playground for approximately 2 minutes on 9/20/2024 during teacher transition. Incoming parent discovered child and brought matter to staff attention. Facility immediately addressed incident and implemented enhanced supervision plan for transitions.

  • Child left unsupervised outside during transition between play yards
  • Breach occurred due to lack of supervision during teacher transition from outdoor to indoor area
