What Happened

The annual inspection found a center that was largely compliant on safety basics but had multiple recordkeeping and equipment issues. The inspector cited a damaged playground structure, outdated staff training records, and medication handling problems, along with a sign-in/out recordkeeping issue.

Observations

  • two plastic rock structures had ripped tarp/handles and dangling ropes that could pose a safety risk
  • staff did not have updated mandated reporter training or proof of flu shots
  • some sign-in/out entries had incorrect dates or times

  • one classroom toilet was not flushing properly
