Case management visit for unusual incident where teacher was observed pushing child, resulting in fall. Type A deficiency cited for violation of child's personal rights.

Observations

  • Teacher S1 observed on parent video pushing child C1
  • Child fell to ground as result of being pushed
  • Staff member S1 terminated following incident
  • All staff subsequently provided with Personal Rights form (LIC613A) and trained on child's personal rights
