What Happened
Annual center inspection found the facility within ratio and generally safe, with clean classrooms, food areas, bathrooms, and outdoor space. Record review identified incomplete child and facility forms, and sign-in/out procedures were not in compliance.
Observations
- Three children were missing information on the Consent for Emergency Medical Treatment form
- Facility roster was incomplete with missing child and physician information
- Sign in/out records were not in compliance, including missing signatures/times and entries for children not present
- Mandated reporter certifications were expired for all staff
- No record of influenza vaccination or waiver was on file for all staff
- Incomplete Identification and Emergency forms were noted
