What the inspector wrote

On December 8, 2023 , Licensing Program Analyst (LPA) Simerjit Kaur arrived at the facility to conduct UNANNOUNCED RANDOM SITE INSPECTION. LPA met with licensee Veronica Cusimano . Upon arrival there were 6 children. Present during the inspection was licensee's finge rprint cleared and associated daughters Isabella Cusimano, and Paris Cusimano. Also residing in the home is licensee’s finger print cleared and associated husband Andrew Cusimano and minor son. Hours of operation for child care are Monday through Friday, 7:00am to 5:00pm. The facility is a one story house. The following was observed during today’s inspection: CAPACITY : The facility operates as a Family Child Care Home (large), which may have a maximum capacity of twelve (12) to fourteen (14) children. At time of inspection, there were six children (three infants and three preschoolers), and daughters/assistants Isabella Cusimano and Paris Cusimano. The facility is in compliance with ratio and capacity limitations. ON LIMIT AREAS (accessible to children in care) : converted garage/day care room, fenced left side and backyard, family room for naps, half bathroom located adjacent to family room. There are ample age appropriate toys that are observed to be safe and in good condition. LPA observed the facility to be clean and in good repair, with heating and ventilation for safety and comfort. The backyard has a fence surrounding the perimeter of the yard. LPA did not observe any dangerous conditions, nor any hazardous or toxic items accessible to children in the ON Limit areas of the facility today. A fire place is located in the family room and it is locked. OFF LIMIT AREAS (not accessible to children in care): Master bedroom/bathroom, right side yard, 2 bedrooms, 1 bathroom. OFF Limit areas are inaccessible by closed and/or locked doors and visual supervision. Licensee is advised to contact Licensing Department so that an inspection can be completed prior to changing an OFF Limit area to ON Limit. There are no pools or any other bodies of water on the premises. There is a covered grill located on the side of yard. All hazardous materials and toxins are kept out of the reach of children. Licensee was reminded that any structural changes or additions to the home must be reported to Community Care Licensing. Continued on LIC 809C EMERGENCY PREPAREDNESS/SAFETY: Facility has a fully charged 3A40BC fire extinguisher. Smoke and carbon monoxide detectors were tested and found to be functioning. First aid supplies are available. A fire/disaster drill was last conducted on 12/4/2023 and meets the six month requirement. Facility has working telephone service. Per licensee, there are firearms in the home, which are kept separately in off limit bedroom. LPA reviewed Emergency Disaster Plan which licensee confirmed to still be current. STAFF/CHILDREN RECORDS REVIEW: Licensee and all adults living and/or working in the home have proper criminal background clearances. Licensee and all assistants have current CPR/First Aid training, which expires on 2/21/2024. Licensee and all assistants' have current mandated reporter training completed 1/8/2022. Licensee and staff are in compliance with immunization law. Children's files were reviewed. Licensee maintains an Infant Sleep Plan for infants up to 12 months old, and an infant sleep log for infants up to 24 months. A facility roster is maintained. LICENSING POSTING (required): Facility license, Notification of Parents’ Rights, Earthquake Preparedness, Emergency Disaster Plan. Licensee was reminded that California law requires Licensees to report unusual incidents and/or injuries to children in care, to the child's parents, and to the Department within 24 hours by phone. Within seven (7) days from the incident, Licensee’s must submit the Unusual Incident/Injury form (LIC 624B) to the Department. LPA informed Licensee that all forms can be downloaded at www.ccld.ca.gov . Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Continued on LIC 809C Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.
