What Happened

The case management visit followed a self-reported incident where a child exited through an open gate and was found by neighbors after several minutes. The licensee had already added a double child safety gate, and a Type A deficiency was cited.

Observations

  • a child left the facility unobserved through a gate left open by a parent
  • the child was found unharmed on a neighbor's porch after about 5 to 10 minutes
  • a double child safety gate had been added at the childcare gate
