What Happened

The family child care home was generally clean and in ratio, with working safety equipment and a large outdoor play area in good condition. File review found missing child paperwork and missing TB test proof for two adults in the home, so deficiencies were cited.

Observations

  • the licensee's daughter and a staff member did not have proof of TB tests
  • some child files were missing LIC 9150 parent notification forms
  • one file was missing a parent signature on LIC 627B emergency medical treatment consent
