What Happened
The annual inspection showed a well-run center with good basic safety systems, but the inspector found multiple maintenance, staff-training, and medication issues. The report also noted an outdated menu, incorrect sign-in/out timestamps, and damaged playground equipment.
Observations
- two plastic rock play structures had ripped handles and dangling ropes
- no staff member had a current Pediatric CPR/First Aid certification
- mandated reporter training was expired or missing for all staff except the site director
- one staff file was missing the LIC 508 health history report
- the snack/menu posting had not been updated since 2019-02-12
- some sign-in/out entries had incorrect dates and times
- prescribed and non-prescribed medications were not all in original containers and some were missing labels or consent forms
