What Happened

Type A violation with $500 civil penalty: Lack of supervision. During lunch-to-nap transition, child (C1) was unsupervised and found under director's desk/table in director's office. Director estimates child was unsupervised for approximately 30 seconds to 1 minute. Staff were managing lunch cleanup, teeth brushing, and nap preparation simultaneously.

Observations

  • Type A violation with $500 civil penalty: Lack of supervision. During transition time between lunch and nap time, child (C1) was unsupervised. Teacher Chao Ping Guan was supervising children brushing teeth, Teacher Aide Janie Bosch was helping children get ready for nap while Director was cleaning tables. Teacher Guan noticed C1 walk past her and initially thought she was going to Director. Noticing child was not with Director, staff redirected supervision roles and Teacher Guan found C1 under director's desk (conflicting reports: LIC624 states under table in office). Director estimates child was unsupervised for approximately 30 seconds - no more than one minute. Child was found and returned to classroom. Incident report conflict: Phone report stated C1 under desk; LIC624 form stated under table.
