What Happened
Type A violation for critical supervision failure. Child left unsupervised on playground on two separate occasions within two weeks (7/17/2024 and 7/30/2024). Both incidents occurred during lunch transitions when child was left in bungalow area unattended.
Observations
- Child left unattended on playground in bungalow area on 7/17/2024 during lunch transition
- Child left unattended in playground bungalow area again on 7/30/2024 during lunch-time transition
- Same child (C1) left unsupervised twice in two-week period while staff transitioned group to classroom
