  • Working telephone on premises
  • TYPE A DEFICIENCY: Plastic bin with 6 bottles of prescribed medication in dining room (on-limit area). Immediately moved to locked bedroom.
  • TYPE A DEFICIENCY: Hazardous items in hallway bathroom (on-limit area): 2 aerosol body sprays, 1 box shower vapor tabs, 1 Lysol spray, 1 fluoride mouthwash. Immediately removed and placed out of reach.
