What the inspector wrote

On July 8, 2025 at 9:15am, Licensing Program Analyst (LPA) Randy Miranda conducted an unannounced case management inspection for the purpose of investigating a reported incident . LPA met with Site Director Ciara Woolery, Lucy Macias, and Executive Director Mimi Albert. Present during the inspection today were 17 staff members and 47 children in care. The purpose of today's visit is to follow up to an unusual incident report submitted by the facility. The facility was toured to conduct a Health and Safety Inspection. The incident occurred on 06/06/2025 and was reported to Licensing on the same day. Per the incident report received from the Center Director, it stated that \342\200\234 On June 6, 2025 around 12:30 PM there were three teachers on the playground supervising 24 kids total. A child name C1 came crying a told staff that he was told to get in the ball bin and the other above referenced kids (C2, C3, and C4) sat on top of the bin for about one minute trapping him inside. The director said she had individual conversations with each child and explain to them the dangers of their behavior. Next she called all the parents of the children involved and updated them on the incident.\342\200\235 During today\342\200\231s inspection, LPA reviewed the above-referenced incident with the Director and conducted a records review to gather additional information. Although one staff member was unavailable for an interview, LPA interviewed two staff members named in the incident report. Based on the information obtained through the records review and staff interviews, LPA determined that there was a lack of visual supervision. The facility was in ratio during outdoor play, and three staff members were present at the time of the incident. However, the timing of the incident was unfortunate, occurring while staff members were in their respective positions and may have been momentarily distracted while attending to other children on the playground. As a result, a Type A deficiency was issued during today\342\200\231s inspection. (See LIC 809D.) In accordance with the Type A deficiency cited during today\342\200\231s inspection, a copy of this report and the LIC 9224 Acknowledgment of Receipt of Licensing Reports must be provided to the parents/guardians of all currently enrolled children, as well as to those enrolling within the 12 months following the date of this report. The LIC 9224 must be signed on the next business day the children are in care and retained in each child\342\200\231s file. A Notice of Site Visit was given and must remain posted for 30 days. This report shall remain on file for three years. Appeal Rights provided. Exit interview conducted and reviewed with Site Director Lucy Macias.
