What Happened
Serious safe sleep violation - 9-month-old infant found sleeping in rocker with blanket, creating risk of breathing obstruction and suffocation. Type A and Type B deficiencies cited; parents must sign acknowledgement.

- Infants not allowed to sleep in rockers due to risk of breathing obstruction
- Blanket on sleeping infant increases risk of suffocation
