What Happened
LPA conducted an unannounced annual inspection at the preschool center with 21 children and five staff present. The site was in ratio and generally orderly, but staff and children's files were incomplete and one staff member lacked fingerprint clearance and required training and immunizations.
Observations
- Staff 03 did not have fingerprint clearance or transfer.
- Required staff were missing Pediatric CPR & First Aid certification.
- All staff files were incomplete.
- All staff were missing immunizations for MMR, Tdap, and influenza.
- All staff were missing mandated reporter training.
- Child 04 and Child 10 files were incomplete.
