What the inspector wrote

On April 5, 2021 at 11:36AM, Licensing Program Analyst (LPA) Catherine Fernandes conducted a tele-visit with Licensee Maureen Vasquez to deliver the finding to the above complaint allegation. Due to COVID-19 the inspection was via telephone. During the course of the investigation LPA Fernandes conducted interviews and reviewed documents pertaining to the complaint. No children were in care during the visit due to spring break. Based on interviews Staff 1 did admit to seeing a scratch on child 1 and forgot to report the scratch to the child's parent. Therefore the allegation is SUBSTANTIATED, the preponderance of evidence standard has been met. Title 22 is being cited on the attached LIC. 9099D. Appeal Rights were discussed An exit interview was conducted Report and Appeal Rights will be emailed and mailed.

This requirement has not been met as evidenced by: Based on interviews staff was unaware of how child 1 got hurt while in care, which can be a potential danger to children in care.
