What the inspector wrote

On 10/08/25 at 1:55 pm, Licensing Program Analyst (LPA) Mario Caro arrived unannounced for a required 3 year inspection and met with the director Michael Vasquez. Present for the inspection were 25 infants in care and 12 additional staff. The entire center was inspected for a health and safety check. Operating hours are Monday - Friday 7:30 am to 6:00 pm. The physical plant was inspected. LPA toured the premises. Indoor space: 4 classrooms, 1 restroom, kitchen, nap room, and outdoor play yards were inspected. Disinfectants, cleaning solutions, and other items that are dangerous to the health and safety of children were stored in places inaccessible to them. Cabinets, drawers, and rooms used for storage were locked. Furniture and equipment were age appropriate and in good condition, free of sharp, loose, or pointed parts. Restrooms for children were observed to be in safe, sanitary, and functioning condition. Changing tables were clean and within arms spand reach of the sinks. Floors were clean and free from tripping hazard. Foods and beverages were stored safely. Food storage areas were clean, free of litter, rubbish, and rodents/vermin. The facility provides snack for the children. The trash cans were in good condition and had tight fitting lids. LPA observed a working Fire extinguisher (2A-10-BC), carbon monoxide detector was tested and is in working order. The built in smoke detectors were tested on 12/16/24. Last disaster drill was conducted on 05/08/25 and was properly logged. Facility does not provide transportation for children, but the owner understands that children cannot be left alone, unattended in parked vehicles. Facility’s License, Parents’ Rights Poster PUB 393, Personal Rights, Activity Schedules, and Menus were observed to be posted. There are no bodies of water accessible to children in care in or around the center. There are no firearms in the facility. There were two babies sleeping in sleep sacs which imposes/imposed a potential risk to the health, safety, or personal rights of clients a Type B deficiency will be cited see deficiency page 809-D. Outdoor Space: Outdoor playgrounds were inspected and observed to be fenced and safe. The play equipment was maintained in good condition and free of hazards. LPA observed toy structures and age appropriate toys and material for children. Shade is provided by way of covered areas. Drinking water is arranged to be readily available to children during indoor and outdoor activities. File Review: Children sign in and out procedures and logs were reviewed. A sampling of Children and Staff files was taken for review. All files contained required documents. There was at least one Teacher with current certification in Pediatric CPR/First Aid present at the facility during inspection. Children's Roster was reviewed, and a copy obtained. On or before March 30, 2018 any person who works in a child care facility shall complete Mandated Reporter training and renew the training every 2 years. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. REPORT CONTINUES ON 809-C. A Notice of Site Visit was given and must remain posted for 30 consecutive days.
