Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Jyoti Saini met with Director Maya Yarbrough for an unannounced Annual Random Inspection. LPA disclosed the purpose of the inspection and was granted entry into the facility by the Director. During the inspection, there were no children present; however, as the facility runs a before and after school program, the sign-in sheet indicated that there were 10 children present for the AM program with three staff members. The program consists of two sessions: 7:00 a.m. to 8:15 a.m. and 12:00 p.m. to 6:00 p.m., Monday through Friday, in Room EDCC #1 (AKA Cubs, ages K\342\200\2231st grade) and Room EDCC #2 (AKA Lions, ages 2nd\342\200\2235th grade) on the Green Elementary School campus. The facility was inspected indoors and outdoors today for health and safety hazards, and the following was observed: There is a working alarm system at the elementary school. The rooms have working smoke and carbon monoxide detectors, fully charged fire extinguishers, and a working telephone available. LPA did not observe any bodies of water during today's inspection. Per the Director, there are no firearms on the premises. All furniture is in good repair. The center has age-appropriate toys and equipment. The children and staff have separate bathrooms, which are safe, sanitary, and free of any hazardous items. Drinking water is readily available through water fountains and water bottles brought from home. A menu is posted for the entire month. The facility serves snacks, and children are required to bring their lunch. LPA reviewed the food storage and observed that all food was in its original packaging and labeled with opened date. Medicines are stored in the kitchen in a locked box. The classrooms have appropriate postings. The last emergency drill was conducted on 08/01/2025 at 10:17 a.m. and is properly logged. The facility utilizes the KT Connect app to sign in and sign out the children. A review of three staff and five children's records was completed. Staff present have criminal record clearances on file. The record review also indicates that the staff members present have current Pediatric First Aid/CPR certificates and Mandated Reporter training. The outdoor play area is free of hazards, and the playground equipment is set on cushioned material that helps absorb falls. LPA reviewed the facility roster, and a copy was obtained.
