What Happened
The center was in ratio and the classrooms, food service areas, bathrooms, and outdoor play space were generally clean and safe. Record review identified multiple staff compliance issues, including missing immunizations, an expired mandated reporter certificate, incomplete staff forms, and missing medication consent forms for two children.
Observations
- Three staff members lacked proof of MMR immunization
- One staff member had an expired mandated reporter training certificate
- One staff member was missing a signed Criminal Record Statement
- One staff member was missing a signed Health Screening Report
- Multiple children's file forms were incomplete or incorrectly filed
- Two children did not have updated medication consent forms for prescribed and nonprescribed medications kept at the facility
