What the inspector wrote

On 11/10/25, at 1:10 pm, Licensing Program Analysts (LPAs) Mario Caro and Paulita De La Cruz arrived on a case management visit while at the center regrading another matter. LPAs met with Principal Madeleine Rogin. Present in care were 62 preschool aged children and 16 additional staff members. The facility had multiple cases of confirmed hand foot and mouth in the preschool center the first confirmed on November 3rd and the second on November 5th. The facility didn't notify licensing to report the outbreak. The Principal stated she wasn't aware they needed to report infectious diseases to licensing which imposed a potential risk to the health, safety, or personal rights of clients. A Type B deficiency will be cited see deficiency page 809-D Exit interview conducted Report and appeal rights were reviewed and provided to Principal Madeleine Rogin.
