On June 11, 2025 at 10:25 AM Licensing Program Analysts (LPAs) Elimika Woods arrived at the facility for an unannounced CASE MANAGEMENT visit for the purpose of clearing a Plan of Correction (POC). LPAs met with licensee Darlene Brownlee. Present during todays visit were three (3) preschool age children. The facility was toured for health and safety inspection. The purpose of the visit was a case management visit to clear the POC dated 0 5/29/2025. Type B-Mandated Reporter training was not completed or renewed by the renewal date. On June 11, 2025 at 10:30 AM the licensee provided the LPA with a copy of her Mandated Reporter training certificate an LPA cleared the POC and provided the licensee a copy of the letter. No deficiency noted during today's visit. A notice of site visit given and must be posted for 30 day. Appeal Rights provided. An exit interview was conducted with licensee Darlene Brownlee.
