What the inspector wrote

On 9/29/25 at 11:35am, LPA Briana Plumboy met with Center Director Shao Liang for a case management visit as a result of receiving an unusual incident report. Present for the visit was 3 staff and 3 preschool age children in care. An incident occurred when a staff member was supervising a child playing outside. The teacher approached the center Director to address a concern regarding a fallen tree branch. The staff who was inside the classroom propped the door open and came outside on the outdoor play yard. It was reported the child went inside the classroom with a family picking up and was outside of the staffs supervision for about 30 seconds. The attached type A deficiency is cited today and must be corrected by the due date. Upon receipt, director shall post this report for 30 days and provide copies of this licensing report to parent/guardians of children in care at the facility and to parent/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 Acknowledgement of Receipt of Licensing Reports should be signed by guardians and placed in each child’s file. An exit interview was conducted. Appeal rights were given and discussed. A site visit notice was posted.

    (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced by on 9/25/25, a child went inside the classroom with a family picking up, while staff was outside on the yard. The child was outside of the staffs supervision for about 30 secondswhich poses an immediate health and safety risk to children in care.
