What Happened
Required inspection identified three critical Type A deficiencies creating immediate health and safety risks: large kitchen knife left accessible to children on counter for over one hour, staircase not gated allowing access to off-limit second floor, and three adult resident children (ages 18, 21, 24) lacking required criminal record clearances.
Observations
- Type A Citation: At 9:15am LPA observed large kitchen knife on kitchen counter. Upon recheck at 10:13am, knife still in same location - not moved. Creates immediate risk to children.
- Type A Citation: Staircase not gated when inspection began, making off-limit second floor accessible to children. Must be gated at all times while children in care.
- Type A Citation: Three adult children residing in home (ages 18, 21, 24) have NOT obtained criminal record clearance, exemption, or transferred existing clearance. Licensee stated awareness of requirement. Children required to submit LiveScan request (LIC9163) by 02/08/2024.
