What the inspector wrote
On May 5, 2026, at 2:00 PM, Licensing Program Analyst (LPA) Elimika Woods met with licensee Rosa Henriquez to conduct an unannounced annual random inspection. The facility operates as a large family childcare home with a licensed capacity of 14 children. At the time of the inspection, there were seven (7) children in care, supervised by one (1) fingerprint-cleared assistant. Of the children present, two (2) were infants, two (2) were school-age, and three (3) were preschool-age. LPA toured the facility with the licensee, both inside and outside, to conduct a health and safety inspection.
The facility is owned and operated by the licensee and consists of four bedrooms, two and a half bathrooms, a living room, dining room, family room, kitchen, backyard, and an attached two car garage. The hours of operation are Monday through Friday, 7:30 AM to 5:30 PM.
The on-limit areas include the living room, dining area, kitchen, family room (childcare room), downstairs bathroom, and backyard. The isolation area is designated in the living room, away from other children in care. The off-limit areas include the entire second level of the home and the garage. These areas are made inaccessible to children by closed and/or locked doors and visual supervision.
The home was observed to be clean, orderly, and well maintained, and is equipped with centralized heating and ventilation to ensure the safety and comfort of children. LPA asked the licensee whether she has firearms in the home, and the licensee stated that she does. At 2:30 PM, LPA inspected the area where the firearms are stored and confirmed that they are properly secured in accordance with applicable regulations. LPA also asked whether the licensee transports children in care. The licensee stated that she does provide transportation and provided a copy of her current California Driver’s License. LPA reviewed the license and verified that it is current.
LPA inspected the cabinets in the on-limit areas, including the living room, dining area, kitchen, family room (childcare room), and downstairs bathroom, and did not observe any hazardous materials or toxins accessible to children during today’s inspection. LPA tested the smoke and carbon monoxide detectors and found them to be functioning properly. The home is equipped with a working telephone, a first aid kit, a pull-down fire alarm system, and a fully charged 3A40BC fire extinguisher, which meets the standards established by the State Fire Marshal. There is also a fireplace in the childcare room that is blocked by furniture to prevent access by children.

At 2:30 PM, LPA inspected the backyard and confirmed that there are no pools, hot tubs, or other bodies of water, in compliance with childcare licensing regulations. The outdoor play area includes play equipment and outdoor furniture, such as an outdoor wicker couch, sink, tricycles, ride-on cars, chairs, a bench, a wagon, a playhouse, and a slide with cushioning underneath to absorb children’s falls. The backyard is fully fenced and free from defects or hazardous conditions. A canopy provides shade and protection from harmful sunlight during outdoor play.
At 2:45 PM, LPA requested and reviewed the files of four (4) children in care and the assistant’s file. All children’s files contained Identification and Emergency Information, Parent’s Rights, and Medical Consent forms. The staff file included required health records, a Statement Acknowledging Requirement to Report Child Abuse, and Employee Rights documentation. During the review, LPA also checked the health records of the licensee, staff, and children and found the licensee to be in compliance with immunization requirements applicable to childcare providers.
The licensee conducts and documents fire and disaster drills at least once every six months, with the most recent drill conducted on February 2, 2026. The licensee has completed Health and Safety training and maintains current CPR and First Aid certification, expiring on October 29, 2026. The facility roster was reviewed and found to be accurate, reflecting both currently enrolled children and those no longer attending the facility. All required licensing documents are posted and visible for public review.

as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
Effective August 1, 2003, California Law requires Family Child Care Home licensees to report unusual incidents or injuries to children in care to children’s parents and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624B). Incidents must be reported within 24 hours by phone, fax, or electronic mail. The licensee was also reminded that Mandated Reporter Training ("General" and "Child Care Providers") is required for all staff and is to be renewed every 2 years by visiting

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

The licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.
