Case Management visit revealed multiple violations: facility out of ratio in morning hours (1 staff for 5 children), infant improperly commingled without waiver, child not signed in (repeat violation), bedding stored improperly, and director serving at multiple facilities simultaneously.

Observations

  • 1 infant commingled in preschool room without waiver for approximately 1.5 hours daily
  • 1 child not signed in (repeat violation from 11/13/2023)
  • From 6:45 AM to 8:00 AM: only 1 staff for 3 preschool children and 2 infants (out of ratio, repeat violation from 4/17/2024)
  • Bedding for napping equipment stacked together on equipment instead of stored separately
  • Director Merat Ayalew listed as director at another facility in San Jose (can only direct one facility)
