What the inspector wrote

Licensing Program Analyst (LPA) Kassandra Medrano met with Facility Representative, Bathsheba "Sheba" Lagasca for an unannounced Case Management Inspection. LPA Medrano was present for an annual inspection for the infant program and found deficiencies within the preschool license. During inspection, LPA was guided through the facility by Sheba who identified herself as the program coordinator. The director, Merat Ayalew, was not present at the site. It was also found during the tour of building and grounds that the facility has marked the "Panda" classroom as off limits area and did not inform the regional office. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Facility representative, Batsheba. California Code of Regulations, Title 22 deficiencies are being cited on the following pages:
