What Happened
Follow-up case management inspection on lead testing results. Two water outlets found to exceed Action Level of 5 ppb. Facility placed signs restricting use and submitted all required documentation.
Observations
- Two water outlets exceeded Action Level of 5 ppb for lead
- One outlet in children's bathroom (Rojo/Amarillo room)
- One outlet in staff bathroom
- Both sinks used only for hand washing (not drinking/food preparation)
- Type A deficiency being cited (documented on 9099-D)
