What the inspector wrote

On Wednesday, May 01, 2024 at 9:45 am, Licensing Program Analysts (LPA) Manel Estoesta and Jaleesa Jackson conducted a Plan of Correction (POC) visit. The Licensee Merat Ayalew arrived at 10:43 am, LPA's met with Merat and explained the purpose of today's visit. At the time of LPA's arrival, present were two (2) Staff for the Infant Component, two (2) Staff for the Preschool Component, 4 infants and 1 preschool child. Facility operates from Monday to Friday 6 AM to 6 PM. LPA's toured the facility to conduct a Health and Safety Inspection. LPA Estoesta informed the Licensee that this report dated 5/1/2024 included a Type A and B Citations which shall be posted for 30 consecutive days as there is an immediate and potential risk to the health, safety, or personal rights of children in care. See LIC 9099 D for the Repeat Violations of Type A and B from the 4/17/2024 visit, new Type B violations, Plan of Corrections (POCs) and Civil Penalties. Also, LPA's informed the Licensee to provide a copy of this licensing report dated 5/1/2024 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. LPA discussed to the Licensee that the Facility will be endorse to a Facility Compliance Plan that will require an annual inspection. LPA gave a copy of PIN 24-03-CCP. This requirement is not met as evidenced by, on LPA's interview, the Licensee claimed that she has assigned a qualified director on her other centers and record says that her name showed a a director on the other siteand on LPA's arrival, on multiple visits, the Licensee was not present, the licensee did not comply with the section cited above in which poses a potential risk to the health, safety or personal rights risk to children in care.

This requirement is not met as evidenced by, 2nd violation, on LPA's observation at 9:50 am, 3 of 5 infants Sign In for the day did not have a full legal signature of the parents and 1 out of 5 infant was not signed in, the licensee did not comply with the section cited above in which poses an immediate risk to the health, safety or personal rights to children in care.

This requirement is not met as evidenced by, 2nd violation, on LPA's observation at 10 am, on LPA's observation, waste container at the Staff bathroom and at the hallway did not have tightfitting cover that was kept on, the licensee did not comply with the section cited above in which poses an immediate risks to the health, safety or personal rights to chldren in care.

This requirement is not met as evidenced by, 2nd violation, on LPA's observation at 9:50 am, Staff Rgbe was by herself supervising 3 infants at the Infant Room, Staff Faye was cleaning in the Preschool Classroom, andthe licensee did not comply with the section cited above in which poses an immediate risk to the health, safety or personal rights to children in care.

This requirement is not met as evidenced by, 2nd violation, on LPA's observation at 10 am, on LPA's observation, infant milk bottle in the refridgerator and infant milk bottle on the counter where the posters were not labeled with the infants name and current date, the licensee did not comply with the section cited above in which poses an immediate risk to the health, safety or personal rights risk to persons in care.

This requirement is not met as evidenced by, 2nd violation, on LPA's observation at 10 am, the Licensee have "Baby's Daily Report Sheet" but the Staff did not document their condition to check for signs of distress, which includes, but is not limited to labored breathing, flushed skin color, increase in body temperature, and restlessness. The licensee did not comply with the section cited above in which poses an immediate risk to the health, safety or personal rights to children in care.

This requirement is not met as evidenced by, 2nd violation, on LPA's observation at 10 am, 1 crib at the infant room had a "jacket", 2 cribs had stains on the sheet and 1 crib mattress was smaller and not fitted in the crib, the licensee did not comply with the section cited above in which poses an immediate risk to the health, safety or personal rights to children in care.
