What Happened
During the unannounced plan-of-correction follow-up, the analyst found the licensee again out of ratio and leaving infants and toddlers unattended while moving between rooms. The visit also turned up an un-cleared adult providing care, expired milk in the refrigerator, and missing posting/receipt documentation tied to the prior citations.
Observations
- Licensee was alone with one infant and seven toddlers on arrival, with another infant brought over from the preschool room.
- Licensee left one infant and seven toddlers alone in the classroom while speaking with preschool teachers.
- Expired milk dated 2/27/2023 was observed in the refrigerator.
- Adult staff member Ingrid Flores Lopez was providing direct care despite not having criminal record clearance.
- Licensee left two infants and seven toddlers alone with the uncleared adult while speaking with the preschool teacher.
- The Non Compliance Conference report was not posted and LIC 9224 acknowledgements were not given to parents.
- Eight unsigned LIC 9224 acknowledgements were found for the prior Type A citations.
