What the inspector wrote

***Amended to change type A to type B for the personal rights violation based on an appeal decision*** On 10/15/2024 at 9:00AM Licensing Program Analyst (LPA) Jaleesa Jackson arrived for an Unannounced Annual/Required Inspection, and met Director Sarah De Sousa. There were 28 preschool aged children and 4 fingerprint cleared staff members present during the inspection. The facility was toured for a health and safety inspection. Hours of operation are Monday through Friday, 7:00AM to 6:00PM. There are three classrooms total for the preschool. LPA toured all of the classrooms. LPA observed that a child asked to go to the restroom and went to the restroom alone. From where S1 was positioned the child was no longer in view. When the child went to the restroom the half door was closed behind her and the stall door was closed as well. LPA asked if the Assistant Director would supervise the child since they were no longer in view of S1 in the classroom. Parent use an app called Procare to sign their children in and out of the facility. There is adequate heating/air conditioning, ventilation and lighting. Drinking water is available inside and outside of the center. Each child has their own cubby where they can store their belongs. Facility uses cots for sleeping and will ask parents to clean bedding and blankets every Friday. The isolation area for sick children is the office, away from other children in care. The facility has smoke detectors, working telephone, and fully charged 2A10BC fire extinguishers and carbon monoxides detectors that are inspected every year by the fire department. The staff's bathroom is separate from the children's bathroom. All sinks and faucets are in safe and sanitary operating condition. There is a changing table available as well for children still in diapers. During today's inspection all hazardous and other potential toxins are made inaccessible to children. This facility provides AM snacks, lunch, and PM snacks. The menu is posted at the front for parents to view. Continued 809-C ***Amended to change type A to type B for the personal rights violation based on an appeal decision*** The outdoor play area was toured and LPA observed there is plenty of shade provided for the children. There's a play structure with a slide that has cushioning to absorb falls. LPA reviewed 5 children's files and 6 staff files. LPA observed at 9:50AM that in S1's classroom that a qualified teacher was not in the classroom. The facility was not aware that S1 was not qualified from due to the prior licensing visits and emails with another analyst. When S2 arrived at 10:00AM they also were not qualified. The director combined the classroom with another to be back in ratio. LPA verified both opening and closing staff have current CPR/First aid training. All staff files reviewed had current mandated reporter training. Disaster drills are being conducted at least once every 6 months and the last one conducted was on 9/20/2024. All required documents are posted in a public accessible area. There were 2 deficiencies cited on today's inspection. See 809-D for deficiencies. Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP . A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Facility Representative Sarah De Sousa.
