What the inspector wrote

Absence of supervision is a Zero Tolerance violation. As a result, a Type A deficiency was issued during today’s inspection. A copy of this report and LIC 9224 Acknowledgment of Receipt of Licensing Reports must be provided to parents/guardians of current enrolled children in care and all children newly enrolled following a 12-month period of this report. The LIC 9224 must be signed within the next business day the children are in care and is to be kept in the children’s files An immediate civil penalty of 500$ is assessed today. An exit interview was conducted with director, Sarah DeSousa. A notice of site visit was given and must remain posted for a period of 30 days. The Facility's appeal rights were reviewed and given to the director, Sarah DeSousa.

Supervision shall include visual observationThis requirement is not met as evidenced by: Based on the interviews and record reviews, C1 and C2 were left unattended on the playground for approximately five to ten minutes, which poses an immediate health and safety risk to children in care.
