Inspector’s narrative

What the inspector wrote

Absence of supervision is a Zero Tolerance violation. As a result, a Type A deficiency was issued during today’s inspection. A copy of this report and LIC 9224 Acknowledgment of Receipt of Licensing Reports must be provided to parents/guardians of current enrolled children in care and all children newly enrolled following a 12-month period of this report. The LIC 9224 must be signed within the next business day the children are in care and is to be kept in the children’s files An immediate civil penalty of 500$ is assessed today. An exit interview was conducted with director, Sarah DeSousa. A notice of site visit was given and must remain posted for a period of 30 days. The Facility's appeal rights were reviewed and given to the director, Sarah DeSousa.

    The department may deny an application for or suspend or revoke any license, registration, or special permit issued under this act upon any of the following grounds and in the manner provided in this act: (c) Conduct which is inimical to the health, morals, welfare, or safety of either an individual in or receiving services from the facility or the people of this state.This requirement is not met as evidenced by: Based on interviews and record review, it was found that there were discrepancies in the information that was provided to the parent regarding an incident. This poses a potential health and safety to children in care.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    Responsibility for Providing Care and Supervision:(a)The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observationThis requirement is not met as evidenced by: Based on the interviews and record reviews, C1 and C2 were left unattended on the playground for approximately five to ten minutes, which poses an immediate health and safety risk to children in care.
