What the inspector wrote

Licensing Program Analysts (LPAs) Jyoti Saini and Kassandra Medrano met with Director, Sarah DeSousa for an unannounced case management inspection. The purpose of the inspection was to discuss an unusual incident that occurred on 2/24/26; which was reported to the department by the center on 2/25/26. Present in the facility are 36 children, and 6 staff including the director. On the day mentioned above, it was reported that two children were left outdoors without adult supervision for approximately 5 to 10 minutes within the facility's fenced playground . The facility was requested to provide video footage related to the incident. The facility stated that they were unable to provide the requested footage. During the investigation, information gathered confirmed that the children were outdoors within the fenced playground without direct supervision for a period of time. Information gathered during the investigation further determined that the information communicated to the parent regarding the incident was not consistent with the information obtained through the investigation. Based on the information gathered, it was determined that inaccurate information was provided to the parent regarding the incident. A notice of site visit was given and must remain posted for 30 days. Exit interview was conducted and report was reviewed with the Facility representative, Sarah DeSousa. This poses a potential health and safety to children in care.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    Responsibility for Providing Care and Supervision:(a)The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observationThis requirement is not met as evidenced by: Based on the interviews and record reviews, C1 and C2 were left unattended on the playground for approximately five to ten minutes, which poses an immediate health and safety risk to children in care.
