Licensing Program Analyst (LPA) Jyoti Saini arrived at the facility for an unannounced Case Management visit for the purpose of clearing Plan of Correction (POC). LPA met Director Sarah DeSousa. The purpose of the visit was to clear the POC dated 03/11/2026. The facility was cited under California Code of Regulations, Title 22, Division 12, Chapter 1, Section 101239(e)(4). During the visit, the LPA inspected the toilet and observed that it had been repaired. The LPA flushed the toilet and confirmed that it was in operable condition. LPA issued the POC clearance letter during the visit. No deficiencies were cited during today’s visit. A Notice of Site Visit was provided and must remain posted for 30 days. Appeal Rights were provided. An exit interview was conducted with Director Sarah DeSousa.
