Licensing Program Analyst (LPA) Jyoti Saini and Licensing Program Manager (LPM) Wynn Norona met with the Director, Sarah Desousa, to serve an Immediate Exclusion notice to an employee identified as S1 on the Confidential Names List. Upon arrival, LPA observed six staff members and 30 preschool children in care. LPA conducted a health and safety inspection of the facility. During the inspection, S1 was served an Immediate Exclusion letter. LPA and LPM personally hand-delivered the exclusion order, titled “Order to Licensee of Immediate Exclusion from Facility,” to the individual. LPA also read the exclusion order to the facility representative, Sarah Desousa. LPA Jyoti Saini and LPM Wynn Norona observed S1 leaving the facility. The Director was advised to disassociate the individual from the facility roster and to submit an updated LIC 500. A review of records today indicates that all staff and other individuals requiring caregiver background checks have the necessary criminal record and child abuse clearances or approved exemptions. Exit interview was conducted with facility director, Sarah Desousa. This report must be made available for public review upon request for three years.
