What the inspector wrote

Licensing Program Analyst (LPA) Jyoti Saini and Licensing Program Manager (LPM) Wynn Norona conducted a Case Management Inspection to review the facility’s request to increase its capacity from 24 to 38 children. The facility has also requested to add Classroom #4 to the infant license, which was initially licensed under the school-age component. LPA met with Director Sarah DeSousa, and the purpose of the inspection was explained. Present during the visit were the Director and four staff members supervising 13 infants. The facility also has two additional components: Preschool (013422425) and School-Age (013422426). During the visit, LPA remeasured the indoor and outdoor spaces. Indoor measurements totaled 1,292.17 square feet. Dividing by 35 square feet per child allows for a maximum capacity of 36 children. The outdoor infant play yard, which is separated from the preschool and school-age yards by a 4-foot gate, measures 44.4 ft x 37.5 ft, totaling 1,665 square feet. The outdoor space allows a maximum of 21 children outdoors. The facility shall create a schedule to ensure that no more than 21 children are taken outdoors at one time. LPA observed proper cushioning (artificial grass and rubber matting) under the infant play structure. There are 12 cribs available for infants under 12 months. A changing table is located within arm’s reach of a sink. The infant room includes a bottle warmer and refrigerator for use. There is designated storage space for each child's personal belongings, as well as mats for infants 12–24 month. All documents requiring posting were posted in a highly visible place for parental review. Director is also reminded that a needs and services plan must be kept on file for each infant and updated at least quarterly. An exit interview was conducted with Director Sarah DeSousa A copy of this report will remain on file for a period of 3 yrs for public review and a copy should be kept in the facility to be provided upon request. A notice of site visit was posted and must remain posted for a period of 30 days The following items are required before a license will be issued: 1. Amended Granted STD850. 2. Updated Application (LIC200A).
