What the inspector wrote

On 09/01/2022 at 9:00am, Licensing Program Analyst (LPA) Christina Uribe conducted an Unannounced Annual Required Inspection . LPA met with director Sukhvinder "Sonu" Sidhu, also present at the time of the inspection were 2 staff & 9 children. The facility is within ratio & capacity compliance today. LPA provided facility representative the Entrance Checklist (LIC 125). The facility was toured to conduct a Health and Safety inspection. The facility currently operates 9:00am-12:00pm, Monday-Friday in one classroom. Classrooms: Classroom was inspected for age-appropriate furnishings, equipment, & adequate storage for children\342\200\231s belongings. LPA observed the cleanliness of floors & surfaces, the presence of a fully functional carbon monoxide detector, smoke detector/fire alarms, and a fully charged 2A10BC fire extinguisher that is accessible throughout the facility. The center is equipped with a fully stocked first-aid kit and available in the classroom. The facility does not provide snack to children in care and also does not have an outdoor play space. Bathrooms: Facility has separate staff and child designated bathrooms. Toilets and faucets are in safe and sanitary operating condition. The children are able to reach the sinks and toilets. Supplies are available to children. Health-Related Services: This facility does provide Incidental Medical Services (IMS), however the facility does not have any medication stored at the facility for children in care at this time. LPA reviewed 10 children\342\200\231s files and 3 staff files. During record review, Personnel Report (LIC 500) and Facility Roster (LIC 9040) were not updated nor available for review. 3 children did not have a Physician's Report (LIC 701) in their file. All children had the incorrect Consent for Emergency Medical Treatment form in their file, instead of LIC 627 form for Child Care Centers, facility was using LIC 627C form for Adult and Senior Residential Facilities. At least one opening/closing staff member has a current Pediatric CPR/First-Aid Certification. Mandated Reporter certificates were reviewed and one staff member only has a current General Training certificate. Director\342\200\231s CPR/First-Aid Certificate is current and expires on 11/19/2022. One staff member does not have a Health Screening Report (LIC 503) form in their file. One teacher present at the facility today does not have transcript records of their education to verify their qualifications. The center is in compliance with sign in and out procedure. Emergency Drills are recorded and performed every six months. Per facility representative, there are no firearms on the premises. All required documents are posted in a publicly accessible area. Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Effective August 1, 2003 California Law requires Child Care Licensees to report unusual incidents or injuries to children in care to child\342\200\231s parents and to the Department of Social Services using the Unusual Incident/Injury Form (LIC 624). Incidents must be reported within 24 hours to the regional office by phone and the written report, LIC 624, within 7 business days. Page 2 of 3 ***Continued on LIC 809C*** No deficiencies cited during today's inspection. Please see list below and attached advisory notes for technical violations issued today: Technical Violation: One staff member does not have a mandated training complete for Child Care Providers (AB 1207). She does however have a current certificate for general training. Technical Violation: One staff member does not have a Health Screening Report (LIC 503) form. Technical Violation: One staff member does not have transcripts available for review. Technical Violation: Personnel Report (LIC 500) neither updated nor available for review. Technical Violation: Three children did not have a signed Physician's Report (LIC 701) form. Technical Violation: All children's files had the incorrect Consent for Emergency Medical Treatment form signed and in their file. The facility was using the LIC 627C form for Adult and Senior Residential Facilities instead of the required LIC 627 form for Child Care Centers. All technical violations issued today are to be corrected and addressed immediately. Director has been issued a plan of correction and due date for the first four technical violations listed above. Failure to meet the deadline will result in the issuance of a Type B Violation deficiency. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the director, Sukhvinder "Sonu" Sidhu.
