Reporting Requirements 101212(d) Upon the occurrence, during the operation of the childcare center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours........ The Unusual Incident was a suspected Children’s Personal Rights Violation dated 1/23/2024, reported by a parent to the Staff. CAPE Management only reported the incident on 2/5/2024 to the Regional Office which is a potential risk to the health and safety of the children in care.
