What the inspector wrote

On June 25, 2025, Licensing Program Analyst (LPA) Simerjit Kaur met with licensee Shruti Agarwal, for the purpose of an Unannounced Case Management Inspection . Present during the inspection, fingerprint clear and associated Rahul Agarwal, Radha Agarwal, and Shikha Agarwal. The facility self reported an unusually incident to licensing on June 18, 2025, and a copy of the Unusual Incident Report (UIR) was reviewed by LPA Kaur. Present during today's inspection were 2 infant age children and 2 staff members. The purpose of the visit was to follow up on an unusual incident reports received and submitted by the facility regarding a child had fracture femur. Licensee is unable to determine the cause of the injury. On June 18, 2025, licensee reported an Unusual Incident to the Regional Office (RO). The summary of the unusual incident is as follows: On 6/17/25 at approximately 3:30pm child was playing with a toy truck and started crying. Child cried off and on for about 20 minutes. Assistants told licensee that child was crying because the toy truck was taken away. Licensee was in the main house when child began to cry. Licensee heard child crying and asked assistants about it. Assistants stated that child was comforted. Assistant rubbed child's back when he started crying. Licensee observed child playing at 3:45pm at that time child was sitting and playing with a toy. Child was not crying. Child was picked up at approximately 4:15pm. When child left he wasn't crying and didn't seem to be in pain. Parent text licensee at 8:55pm asking if something happened in care and if licensee noticed anything strange with child's walk as child had been crying at home and not wanting to walk. Parent also reported that child was lethargic. Child has a medical appointment 6/18/25 at 11:45am. Child is not in care today. Licensee stated that camera that records in the home was not working. LPA Kaur conducted interviews of staff. There are no deficiencies being cited today. This report shall remain on file for 3 years. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with licensee Shruti Agarwal.
