What the inspector wrote

On 11/4/25, LPA Briana Plumboy met with Director Anu Patel for an unannounced random inspection. The center and playground were toured to conduct a Health and Safety inspection. There were 24 preschool age children present during the visit. Also present for the visit was 6 fingerprint clear and associated staff. The center currently operates Monday through Friday from 8:00am until 6:00pm. The center also has a Heritage school on-site. Heritage school utilizes the upstairs classrooms which preschool and toddler children do not have access to. The program consists of 4 rooms. 2 Preschool classrooms are located in the 32462 Alvarado Blvd building. The 2 Toddler-Option classrooms are located in the 32464 Alvarado Blvd building. The center currently possesses two waivers. One waiver is for the electronic sign in/out system utilized by the center, and the second waiver is for the facility's playground and the amount of children allocated on the playground at one time. There are cubbies for children's belongings. There is adequate furniture, toys and activities, which are in good condition today. The heating and lighting is adequate. There is drinking water and cups readily available to children during today's inspection. There are combined bathrooms for boys and girls. There is a separate bathroom for staff which is located off the left of the main entrance to to center and another staff restroom located in the toddler building. There are no bodies of water, or free standing water, accessible to children during today's inspection. Per Director, there are no firearms on the premises of the child care center. The food preparation area is clean. There is a full kitchen located inside classroom #2, and there is a barricade to prevent access from children in care. The am/pm snack and lunches are brought from home, and the center provides lunch to families who request it, or the children may bring lunch from home. There are no cleaning supplies stored with food during today's inspection. The center has emergency snacks available for children in care. There are mats and sheets available for children's use during today's inspection. The playground is fenced and has cushioned material and cement as a surface. There are anchored play structures present during today's inspection. All required documents are posted for public review. The center is in compli ance with the sign in and out procedure. Disaster drills are being conducted at least once every 6 months, and the log indicates the last earthquake drill was conducted on 9/9/25, and the last fire drill was conducted on 9/23/25. See 809-C for continuance The center is equipped with a first aid kit, working telephone, working carbon monoxide detectors, working smoke detectors, pull down fire alarm and fire extinguishers. LPA Plumboy obtained a copy of the roster. Center Director provided LPA Plumboy with a copy of Lic.500 which is the facility (Personnel Report). The Director is aware at all times when children are in care a staff must be present who is CPR/First Aid certified, and during today's inspection LPA Plumboy verified all staff at the center have current CPR/ First Aid. Incidental Medical Services (IMS) policy was discussed. This facility provides IMS to children in care. The facility is following and have developed IMS plan on file. When any changes to the IMS plan is made, an updated Plan for Providing IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm .” Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed childcare center. A civil penalty of $100.00 minimum/day up to $3000.00 maximum per day/per person will be assessed if this regulation is violated. Director was reminded that California Law requires licensed Child Care Centers to report unusual incidents or injuries to children in care to child's parents and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624). Incidents must be reported within 24 hours by phone, fax, or electronic mail. LPA informed the Facility Representative that all forms can be downloaded at www.ccld.ca.gov and encouraged the Facility Representative to email childcareadvocatesprogram@dss.ca.gov to be included in the Child Care Quarterly Updates distribution list. Director was also reminded that Mandated Reporter Training ("General" and "Child Care Providers") is required for all staff and is to be renewed every 2 years by visiting www.mandatedreporterca.com . There were no deficiencies were cited for today's inspection . Appeal rights and a notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.00. Exit interview conducted and report was reviewed with center director, Anu Patel.
