What the inspector wrote

On 8/11/21 at 12:00pm, Licensing Program Analysts (LPAs) Melissa Domantay and Melissa Guirit arrived at the facility for an announced prelicensing inspection. LPAs met with applicants Rebecca Lidow and Kelly Johnston, and there was no one else present. The applicants submitted an application for a preschool license at this facility. The facility will operate Monday-Friday 8:00am-5:00pm in 1 classroom and Rest Area Room, and Kitchen that will be ON LIMITS for children in 1 building. Staff bathroom inside building is OFF LIMITS to children. Parent board is inside classroom by entrance. LPAs toured all areas to be used by children, and the measurements are as follows: INDOORS: 863.43 square feet= 24 children OUTDOORS: 3,652.98 square feet= 48 children A fire clearance, with a capacity of 22 preschoolers was received from City of Berkeley Fire Department. LPAs observed that all classrooms have sufficient lighting, heating and ventilation for the safety and comfort of children and staff. The floors and surfaces were clean and in good condition. LPAs observed an ample supply of age appropriate activities, equipment, and furniture that is in good condition. The climbing structures in the upper and lower outdoor play yards have tan bark as sufficient cushioning underneath to absorb a fall. The outdoor play area is fully fenced and has an ample supply of toys and activities. LPAs did not observe any defects or dangerous conditions in the outdoor play areas. There are 2 toilets and 2 sinks available for children, and they are in sanitary and operational condition. There is a separate bathroom for staff. The Rest Area Room will be used for isolation of sick children. The facility is equipped with 3A40BC fire extinguisher, centralized smoke and carbon monoxide detectors, telephone, and first aid supplies. LPAs did not observe any hazardous items, toxins, bodies of water or medication accessible to children today. The facility will provide snacks and families will bring lunch from home for children. Children will have individual water bottles, which will be refilled as needed, so there will be water available both indoors and outdoors. The COVID-19 Assessment has been completed. If This facility plans to provide Individual Medical Services – IMS. A Plan of Operation that includes IMS must be submitted to the Department. Marisa Ramirez is a fully-qualified preschool director with a site supervisor permit and has completed 16 hours of health, and safety training, lead poisoning prevention training, and pediatric CPR and First Aid. Mandated reporter and appeal rights, civil penalties, unusual incident reporting and fingerprint requirements were discussed today. Licensee is also being informed of the web address ( www.ccld.ca.gov ) for downloading child care forms, and the director is encouraged to email ChildCareAdvocatesprogram@dss.ca.gov to be included in the Child Care Quarterly Updates distribution list. The director is also reminded that mandated reporter training is required for all staff and is to be renewed every 2 years at www.mandatedreporterca.com . Zero Tolerance policies were explained. The center was found to be clean, safe, sanitary, and in good repair. A license for 22 preschool children will be effective today 8/11/21. This report will remain on file for 3 years. An exit interview was conducted, and a copy of the report and appeal rights were provided to applicants.
