What the inspector wrote

On October 2, 2025, Licensing Program Analyst (LPA) Jyoti Saini conducted an unannounced case management inspection at the family child care home in response to an Unusual Incident Report submitted by the licensee on September 26, 2025. During the inspection, the licensee was supervising 11 children\342\200\224two infants and nine preschoolers. Based on the information gathered, the incident occurred on September 24, 2025, when Child #1 (C1) was left unattended for approximately 1\342\200\2232 minutes on the diaper changing station while the licensee packed C1\342\200\231s belongings in preparation for pickup. During this time, Child #2 (C2) approached C1 and bit and stretched the child, resulting in a violation of C1\342\200\231s personal rights. As a result, a Type A deficiency was issued during the inspection, citing California Code of Regulations, Title 22, Division 12, Section 102423 ((a)(2), as documented on the attached LIC 809D. A copy of this report and the LIC 9224 Acknowledgment of Receipt of Licensing Reports must be provided to the parents/guardians of currently enrolled children and all newly enrolled children for a period of 12 months following the date of this report. The LIC 9224 must be signed within the next business day that children are in care and retained in each child\342\200\231s file. A Notice of Site Visit was issued and must remain posted for 30 days. The facility\342\200\231s appeal rights were reviewed and provided to the Licensee Lei Sara & Wong Anthony.
