On October 30, 2025, at approximately 7:45 AM, Licensing Program Analyst (LPA) Elimika Woods arrived unannounced to conduct a Plan of Correction (POC) inspection to verify that the deficiency cited on October 22, 2025, had been corrected. LPA met with Licensee Shibani Sahur. Present during today\342\200\231s inspection was the licensee\342\200\231s school-age son. No day care children were present during today\342\200\231s inspection. During the annual random inspection conducted on October 22, 2025, the licensee\342\200\231s assistant did not have proof of immunization records (MMR) on file. Today, the licensee provided LPA with a copy of the updated MMR immunization record for the assistant (S1). LPA verified the documentation and cleared the POC cited on October 22, 2025. A POC Cleared Letter was provided to the licensee. No deficiencies were cited during today\342\200\231s inspection. A Notice of Site Visit was issued and must remain posted for 30 days. An exit interview was conducted, the report was reviewed, and Appeal Rights were provided to Licensee Shibani Sahur.
