What the inspector wrote

On Tuesday, March 18,2025 Centralized Application Bureau (CAB) Licensing Program Analysts (LPAs) M. Domantay and Diana Campos met with Applicant Rickey Hayes and Director Lisa Ross for the purpose of a pre-licensing inspection. Applicant requests for a single license facility for a Preschool license to serve 30 Preschool children and 20 School age children. The fire clearance was granted and approved on 4/19/2024 from the Alameda City Fire Department. LPAs toured and inspected planned indoor or outdoor areas of the preschool and school age programs. Preschool and school age indoor and outdoor areas are not fully set up to be measured or for final inspection. Per Applicant Rickey and Director Lisa will work on setting up child care areas sufficiently before final pre licensing is conducted with set up indoor and outdoor age appropriate equipment and furniture. LPAs notified Applicant and Director, LPA will conduct inspection once all indoor and outdoor preschool and school age areas are fully set up for inspection and child care. Exit interview conducted and report was reviewed with the Applicant, Rickey Hayes. The following items are required before a license will be issued: 1. Applicant must submit updated LIC200A showing 30 preschoolers and 15 school age children. 2. Applicant must must install locks on all storage doors and kitchen doors to ensure they are inaccessible to children. 3. Applicant must ensure doors too preschool bathrooms are removed or latch is installed to hold doors open. 4. Applicant must sweep, shampoo carpets, and wipe windows and walls. 5. Applicant must install additional shading in the preschool outdoor play yard. Page 1 6. Applicant must repair wired metal fencing in the preschool outdoor play yard to ensure it is closed in and no open gaps where children can get through and install paneling on the wired metal fencing. 7. Applicant must install barrier/gate separating preschool outdoor play yard and storage area. 8. Applicant must remove all furniture and equipment that are not in good repair from the preschool outdoor play yard. 9. Applicant must repair preschool bathroom sink in the hall due to leaking. 10. Applicant must install child gate in front of stairs in the hall. 11. Applicant or Director must submit Site Supervisor permit showing Director's name. Page 2
