What the inspector wrote

Centralized Application Bureau (CAB) Licensing Program Analyst (LPA) M. Domantay, LPA Diana Campos, Karina Canela met with Applicant Rickey Hayes and Director Lisa Ross for the purpose of a pre-licensing inspection. Applicant requests a single licensed child care center for a preschool program ages 3 years to first grade entry capacity 30 and school age program ages 4.9 to 12 years capacity 20, license to serve a total of 50 children. The program will operate Monday through Friday from 6:00 a.m. to 6:00 p.m. The fire clearance was granted on 4/19/2024 from the Alameda City Fire Department. The facility will provide snacks and lunch. LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. INDOOR ACTIVITY SPACE: There are 2 indoor areas for the preschool program, classroom 1 and classroom 2. There is 1 indoor area for the school age program, classroom 3. LPAs observed a sufficient amount of equipment, toys, tables, chairs, cubbies, and cots. LPAs observed a first aid kit in each classroom. Medications will be stored in a red backpack hanging in each classroom per Applicant and Director, Director will send a photo of medication backpacks to LPA. LPAs observed cleaning disinfectants are appropriately stored and inaccessible to children. Applicant stated there are no poisons or firearms on the premises. LPAs observed a functional carbon monoxide detector in classroom 2. LPAs observed a manual sign-in/sign-out system. Children who become ill during the day will be isolated in the Director's office and will use the staff restroom, if necessary. Page 1 LPA measured all classrooms/areas for the preschool program and school age program. The total classroom space for the preschool classroom contains a total of 613.95 square feet, which will not accommodate Applicant's request for 30 preschool children. The total classroom space for the school age classroom contains a total of 533.20 square feet, which will accommodate Applicant's request for 15 school age children. There is 2 toilets and 2 sinks for preschool children, one preschool bathroom is located in between the preschool classrooms 1 and classroom 2, second preschool bathroom is located in the hall and has a latch on the door to ensure it is kept open for supervision. There is 1 toilet, 1 urinal, and 1 sink for the school age children in the hallway, and a separate private restroom for the staff is located in the church. Individual measurements are recorded on the Capacity Worksheet (LIC 9024). OUTDOOR ACTIVITY SPACE: LPAs measured all outdoor areas for Preschool and school age program. The outdoor play area is fenced with a wired metal fencing that is at least four feet tall, one side has mesh paneling between the fencing. LPA observed a sufficient amount of equipment and toys. There is a sandbox in the play yard that was observed to be maintained and free of debris, no sand was present during visit. There is no play structure present in the outdoor play yard area. There are no bodies of water on the premises. LPAs reminded Applicant and Director water tables must be emptied when not in use. There is a sufficient area where shading is provided by a canopy in the play yard. The total outdoor space for the Preschool and school age program contains a total of 3953.26 square feet, which will accommodate Applicant's request for 50 children. Individual measurements are recorded on the Capacity Worksheet (LIC 9024). LPAs notified Applicant will need to submit a request for a waiver for the outdoor play yard to be shared with the preschool and school age children program with a set schedule in place. LPA discussed the following: 100% supervision is required at all times, including in the bathroom; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. LPA discussed with Applicant any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days. Page 2 Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. This facility plans to provide Incidental Medical Services \342\200\223 IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. Page 3 Exit interview conducted and report was reviewed with the Applicant, Rickey Hayes. The following items are required before a license will be issued: 1. Regional Manager (RM) final file review. 2. Alarms to be installed on the school age doors leading outside. Applicant must provide photos and videos of alarm system. 3. Waiver Request for Preschool and school age program play yard to be shared between programs with a rotation schedule in place must be submitted by Applicant. 4. Partition of at least 4 feet tall must be installed in the school age classroom between classroom space and stage area. 5. Photos of the medication backpack in each classroom. Page 4
