What the inspector wrote

On October 9, 2025, Licensing Program Analysts (LPA) Lorraine Dacanay Breaux met with Licensee Farishta Qadar for an Unannounced 1- Year Annual Inspection . Present for this inspection were twelve (12) preschool age children in care, and 1 (one) fingerprint cleared staff/assistant, Sara Moqim. The home was toured to conduct a Health and Safety Inspection inside and out. Facility operating hours will be Monday thru Friday 8:00 AM to 05:30 PM. The home is two story. The home is clean and orderly with heating and ventilation for safety and comfort. ON LIMIT AREAS: Day care classroom/family room, bonus room (rear), kitchen and half bathroom on the first floor near the laundry (near the garage). OFF-LIMIT AREAS : Entire second level, living room, dining room, covered patio/deck, the garage which will be inaccessible by closed and/or locked doors, safety gate, and visual supervision. ISOLATION ARE: kitchen area away from the other children in care, until parents arrive. OUTDOOR PLAY AREA: Located in the backyard and is fully fenced and in good condition with no hazardous materials accessible to children in care. There is a shed that is locked. There are no other pools, hot tubs, or any other bodies of water present in the on-limit areas during today\342\200\231s inspection. Licensee confirmed there are no pool/bodies of water at the home. The home has a working smoke detector, working carbon monoxide detector, first aid kit, telephone, and fully charged 2A10BC fire extinguisher located in the family room. Per licensee, there are no firearms in the home. Per licensee does not provide transportation and confirmed that she resides in the home. The licensee completed the Health and Safety training, CPR/First Aid certification expires on 02/11/2026. The licensee is in compliance with the immunization laws and has completed the mandated reporter training on 02/23/2024. Licensee is reminded of their responsibility to renew CPR/First Aid and Mandated Reporter certificates every two years. The licensee conducts and documents fire and disaster drills twice a year and the last conducted drill was on 06/13/2025. All required forms are posted and visible for public review. Records: At 12:30PM LPA requested and reviewed children and personal files. LPA reviewed 3 children\342\200\231s files and personnel records. All children\342\200\231s files contained Identification & Emergency Information, Parent\342\200\231s Rights, and Medical Consent forms. During the record review, LPA checked the health records of the licensee and found that the licensee is in compliance with immunization requirements applicable to all childcare providers. Criminal Record Clearance: Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. MyChildCarePlan.org: Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Page 2 of 3 ***Continued on LIC 809C*** Safe Sleep: LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS): Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Unusual Incident/Student Injury Report: Effective August 1, 2003 California Law requires Child Care Licensees to report unusual incidents or injuries to children in care to child\342\200\231s parents and to the Department of Social Services using the Unusual Incident/Injury Form (LIC 624). Incidents must be reported within 24 hours to the regional office by phone and the written report, LIC 624, within 7 business days. Megan\342\200\231s Law: During the exit interview, the licensee, Farishta Qadar confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. No deficiencies note issued during today's visit: A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal rights were given and reviewed.
