What Happened
Family child care home with 5 children including infants. Inspector observed two pack-and-plays with improper bedding not meeting safe sleep requirements. One child's file missing required parental rights documentation.
Observations
- Pack and play #1 has very loose fitted sheet — does not meet tight-fitted sheet requirement
- Pack and play #2 has fleece blanket wrapped around it instead of tight-fitted sheet — does not meet safe sleep requirements
- One child's file missing LIC995 (Parental Rights form)
- Licensee's brother lacks proof of tuberculosis test
